- Group Registration Form:Use this form to sign up your group for the remainder of the assignments. Both group members should submit the form (to confirm that both members wish to work together).

Special Consideration: If you are unable to complete homework or if you miss the midterm test due to major illness or other circumstances completely outside of your control, have your doctor complete an official U of T Medical Certificate and submit it to your instructor as soon as possible.
IMPORTANT: Notify your instructor as soon as possible if you find yourself in such a situation. It is always easier to resolve situations earlier rather than later.
- Remark Form:Use this form to request a remark of an assignment or midterm if you feel a mistake has been made. Please be clear and concise. If we can't understand what you're asking for, we can't do the remark.
- St. George Grace Day Request form: Use this form to request one or more grace days (or to cancel a previous request). For students on the St. George campus only.
